Professional baseball player Casey Kotchman credits his amazing 2011 season to improved eyesight.

Despite the effectiveness of Pap tests in preventing deaths, the most recent NCI statistics (2005) indicate that more than 20 percent of women aged 18 and older had not had a Pap test within the past three years.

According to the National Cervical Cancer Coalition, while routine administration of Pap tests is the best method to detect cervical cancer at an early stage, vaccines have the potential to protect women from the disease by targeting cancer-causing types of human papillomavirus (HPV). HPV, a virus transmitted through sexual contact, is the single known cause of cervical cancer.

Women with overly large breasts, or macromastia, have an increased risk of breast cancer, says Dr. Grant Stevens, a renowned board-certified plastic surgeon and breast reduction surgeon at Marina Plastic Surgery Associates. The reason is not clear, he says, but it may be linked to obesity, where breast cancer rates are statistically higher, or a delayed diagnosis of breast cancer, which is more common in large-breasted women.

According to People magazine, comedian Wanda Sykes underwent breast cancer surgery in August after cancer was discovered as a result of her breast reduction surgery earlier this year.

During a September episode of The Ellen DeGeneres Show, Sykes told DeGeneres that when breast tissue that had been removed during her surgery was routinely sent to the laboratory, ductal carcinoma in situ was detected.

“We have been carefully watching the literature over the years linking macromastia to breast cancer,” Dr. Stevens said. “In addition to the obvious problems big breasts bring, such as back strain, bra straps cutting into the shoulders and difficulty exercising, cancer is seen more often in women with large breasts. Ms. Sykes experience is a classic example of the difficulty in diagnosing cancer in large-breasted women.”

Dr. Stevens did not treat Ms. Sykes, but he has had experience with similar cases.

“In addition, we find that women with large breasts are also very self-conscious about them. We have done many breasts reductions, and the women are overwhelmingly thrilled with the freedom of a smaller breast size,” Dr. Stevens says.

The risk of breast cancer should be a reason for large-breasted women to consider reduction surgery, said Dr. Stevens, founder of Marina Plastic Surgery.

“There have been multiple studies to compare breast cancer rates after breast reduction surgery. – The estimated incidence of colon cancer in France is about 34 000 new cases per year. In 20 years the incidence has increased by about 40%.
– Colon cancer reaches more particularly the population over 50 years with a prognosis of 5-year survival of only 55%
– The incidence of mortality from colon cancer in France is 18.5 per 100 000 inhabitants. 
– Colon cancer is characterized by a slight male predominance with a sex ratio near 1.5.

The natural history of disease: Subjects at risk

                    1 – average risk patients

These are the topics within the general population, individuals with no family history of colon cancer or other cancers encouraging. In this population, the largest, colon cancer is rare before age 50 with a rapid increase with age, the average age of diagnosis was 69.5 years for men and 72.8 years for women.

                    2 – High-Risk Subjects

They are of different origins:

Subjects with one or more first degree relatives are diagnosed with colorectal cancer
Studies suggest that individuals with one or more first degree relatives diagnosed with colorectal cancer have a risk of suffering from this cancer doubled compared to the general population. It seems that the risk is especially increased as age of patient is low. The Consensus Conference recommended a screening colonoscopy in individuals with one parent was suffering from colorectal cancer before age 65. The discovery of polyps and their resection requires a test, after 3 years, if a period of 5 years is more than enough before the second check.

Subjects with a family history of colorectal adenoma
The presence of colorectal adenomas is a risk factor for colorectal cancer. The relative risk of parents and siblings of subjects with adenomas compared to the spouse is around 1.8. This risk is increased if the related subjects with adenomas are under the age of 60. A study in Côte d’suggests that the risk of colorectal cancer is related to the size of the adenoma: it is not different from that of controls in the first degree related subjects with adenomas less 1 cm in diameter, whereas it is multiplied by 2.1 in relatives of subjects with adenomas larger than 1 cm in diameter.

Subjects with a personal history of colorectal tumor
The risk of colorectal cancer is higher, multiplied by 3.6 in patients with an adenoma or villous structures of more than one centimeter in diameter and multiplied by 6.6 if the adenomas are multiple.

Subjects with inflammatory bowel disease
Inflammatory diseases of the colon are exposed to high risk of colon cancer. 
- Ulcerative colitis : the risk of developing colorectal cancer is much more important in case of pancolitis that in case of limited left colon. It is not increased significantly in cases of proctitis.
- Crohn’s disease : some studies suggest that the risk of colorectal cancer is higher if Crohn’s disease was diagnosed before age 30 and was initially extended.

Other risk groups
Women with ovarian cancer or uterine cancer have a risk of colorectal cancer a bit higher than the general population. The colorectal cancer risk is multiplied by 2 only among women with breast cancer when it is diagnosed before age 45 and has at least 10 years of evolution.

                    3 – Subjects with high risk

These subjects are a group with two genetically determined diseases: 

- syndrome of hereditary colorectal cancer without polyps or Lynch syndrome : this syndrome, the mutated genes belong to the family of genes involved in mismatch repair system of DNA.
- familial adenomatous polyposis (FAP) : less than 1% of colorectal cancers are due to FAP.This disease is characterized by the development of hundreds of adenomatous polyps lining the intestinal mucosa.

Screening

Screening is based on risk level and type of population.

– Mass screening is for 12 million people in France without a risk factor for colorectal cancer and, because they are aged 50 or older, have an average risk of developing the disease. 
– Only tests for occult blood in stools fit this mass screening, and among them, only the Hemoccult was seriously evaluated. 
– The practice of large-scale biennial Hemoccult could reduce colorectal cancer mortality in the population subjected to screening 15 to 20% if the turnout is high during the first season (more 50%) and that these subjects continue to participate massively in successive campaigns, and virtually all subjects with a positive test had a colonoscopy.

Elements of diagnosis

                    1 – Colonoscopy

– Total colonoscopy is the ideal way of screening for colorectal cancer in patients at high risk. 
– It’s the most effective endoscopic examination on the diagnosis. It is more a treatment technique in case of discovery of a polyp as it allows their removal. The malignant polyp represents only 5-6% of all adenomas removed.
– Colonoscopy requires preparation of good quality will be achieved through the use of PEG 4000 in various forms. The examination must be performed under anesthesia.
– A polyp becoming cancerous ten, it is assumed that all polyps should be removed

                    2 – Other screening techniques

– Rectal examination: is limited to tumors located in the last 10 cm of colon
– Sigmoidoscopy: to view the terminal portion of the colon which is home to three quarters of tumors
– Imagery currently under consideration for screening MRI, ultrasound scans 
– Exploration radiological barium enema. The progress of a barium fluid is followed by fluoroscopy.
– Pathology: The most common histological type is adenocarcinoma lieberkumien (adenocarcinoma) typical or atypical (colloid mucous or anaplastic).

Knowledge of prognostic factors of cancers of the colon can specify the role of different treatments in the context of a multidisciplinary management.
